Does creatine cause hair loss in women?

No. There is no study showing that creatine monohydrate causes hair loss in women, and none showing it in men either. The belief traces back to a single 2009 study of twenty college-aged male rugby players, which measured a hormone ratio in blood and never assessed hair at all. Reviews of the literature since have repeatedly examined the claim and found no evidence behind it.

The short answer

In the decades creatine has been studied, hair loss has not turned up as an outcome. Not as a finding, not as a side effect reported by participants, not in the safety reviews. A supplement taken by millions of people for thirty years would not hide an effect on hair this well.

What exists instead is one study, one measurement, and a great deal of repetition.

Where the myth came from

In 2009, researchers in South Africa gave twenty college-aged male rugby players creatine — a loading dose for a week, then a maintenance dose for two more — and measured testosterone and dihydrotestosterone, or DHT, in their blood.

DHT is the androgen implicated in male pattern baldness, in people who are genetically susceptible to it. The study found DHT rose during loading and stayed elevated above baseline through the maintenance period, shifting the ratio between the two hormones.

That is the entire basis of the claim. The finding was about a hormone in blood, in twenty men, over three weeks.

What that study did not show

Four things are worth being precise about.

A raised hormone marker in one small trial is a reason to run more studies. It is not a finding about hair, and it was never presented as one.

What the reviews say

This question comes up often enough that the researchers have addressed it directly. A 2021 review in the Journal of the International Society of Sports Nutrition set out to examine the common misconceptions about creatine, hair loss among them, and concluded that the evidence does not support a link. The society's position stand on creatine safety, which surveys the broader literature, does not list hair loss among the adverse effects.

The systematic review looking specifically at adverse outcomes in women taking oral creatine monohydrate likewise found no increase in adverse events.

Why the myth spread anyway

The chain of reasoning is easy to follow and wrong at the last step. DHT is genuinely implicated in male pattern baldness. The 2009 study genuinely found DHT rose. Therefore, the reasoning goes, creatine causes baldness.

The step that fails is the one connecting a hormone level within the normal range to an actual outcome in actual hair. Hormone markers move for many reasons and most of those movements never produce a visible effect. Establishing that creatine causes hair loss would require studying hair in people taking creatine, over time, against a control group. That study does not exist, so the claim has never been tested — let alone supported.

It also spread because it is a good story. A supplement that makes you strong but costs you your hair has a shape people remember, and it circulated through forums and social media for over a decade with the qualifiers stripped off. By the time it reached most women, it had become "creatine causes hair loss" with no mention of twenty men, three weeks, or a blood test.

What happened when others looked

Studies examining creatine and androgen levels since 2009 have generally not reproduced the DHT finding. That is the normal fate of a single small result, and it is why the field does not treat one study as settled.

Meanwhile the safety literature has kept accumulating, and hair has not appeared in it. The position stand of the International Society of Sports Nutrition, which surveys the safety evidence, does not list hair loss among the adverse effects. The 2021 review that examined the misconceptions specifically — including this one — concluded the evidence does not support a link. A systematic review of adverse outcomes in women taking oral creatine monohydrate found no increase in adverse events of any kind.

Three independent looks at the literature, none of them finding what the myth claims.

If you are worried anyway

The fear is understandable even when the evidence is clear, and there is a reasonable way to handle it rather than just being told not to worry.

Get a baseline before you start. If you are anxious about your hair, note where you are now — how much comes out when you wash, what your part looks like, whether you have photographs. Hair sheds constantly and people notice their own shedding most in the weeks after they start looking for it, which is exactly when a new supplement gets blamed.

If you also want the underlying picture, ask your doctor for ferritin, thyroid function and vitamin D. Those are the usual findings in women who present with thinning hair, and they are worth knowing regardless of whether you ever take creatine.

If your hair is thinning, look elsewhere first

Hair loss in women is common and it usually has a cause worth identifying. The frequent ones are low iron stores, thyroid dysfunction, PCOS, the postpartum shedding that follows pregnancy by a few months, rapid weight loss or severe calorie restriction, chronic stress, and female pattern hair loss, which is genetic.

Several of those are straightforwardly treatable once found, and a blood test is usually where that starts. Stopping a supplement that has never been shown to cause hair loss, and leaving an iron deficiency or a thyroid problem unexamined, is the genuinely costly mistake here.

Talk to a doctor or a dermatologist. That is a real answer in a way that a website is not.

The bottom line

One study, twenty men, three weeks, a hormone marker that stayed within normal range, and no measurement of hair whatsoever. That is what the fear rests on. Sixteen years of subsequent research has not built anything on top of it.
